Please be advised that certain homeowners have become aware of various construction-related problems in or around their units. This Questionnaire was drafted to obtain general background information from you as to the problems you have noticed. Please answer all questions to the extent they apply to you, stating “yes”, “no”, "unknown" or "not applicable" ("N/A"), if necessary. Your survey answers will be provided to David A.
