Consents

It is part of the DC's responsibility to check the ICF for completion and accuracy.

You can check the ICF for completion and accuracy by accessing the signed document in Epic.

PTrax provides details on which ICF version was signed, when it was signed, and who obtained consent.

You can find the ICF in IRBe, PTrax and the Research Protocol Catalog (Cancer only)

ICF stands for Informed Consent Form.

The CRC/ACRC/PI can sign their portion of the ICF before the participant.

The participant doesn't need to sign the Pre-Screening ICF for us to collect medical history data.

It is not our problem if something on the ICF is missing or incorrect.

If there's a mistake, you can cross it off or use white out as long as you initial and date it.

DC's can obtain consent.

If a participant declines an optional consent, they are also declining participation in the study as a whole.
