The spelling and grammar for the open-ended question will not affect the results of this study and should not exceed 100 words. We ask to please refrain from including any personal information such as names and addresses. 
 
The questions featured on the questionnaire will be answered using a sliding rating scale ranging from 1 to 7*. Details of the rating system are as followed, 
  • "1' indicating least in agreement
  • "4" indicating neutral 
  • "7" indicating most in agreement
We do ask that you please answer the questions as accurately and honestly as possible.
