We can append GC modifier in protocol visit.

Patient has came for chest pain MD documented cardiac arrest has final impression, and consulted cardio team and admitted on 12/15/2023 and US performed in 12/16/2023, can can code US without 25,26 modifier.

RSV can be pdx when no other diagnosis documented in MD Documentation.

Incision and drainage (Paronychia) left index finger performed in ED with gauze in ED assign CPT for the procedure

While assigning Reduction CPT we have to append 54 mod with procedure while assigning 25 MOD in ed level .

We have to use RT/LT modifier while coding Chest tube procedure instead of 52 or 50 MOD.
