Supervised Visitation Survey

Please answer the following questions only if you have utilized our grant funded services (free service). Keep in mind we are funded a specific amount each year that has to be spread out over the full year to ensure an equal amount of visitation each month. We are only able to expand services by using our paid visits at this time. We are working to find additional funding to expand the free service. These surveys are sent to our funders directly to show them if the free service is a need or not. So please answer honestly, all submissions impact whether or not we receive the funding each year based on the need in our area. Any suggestions are helpful; I just wanted to make note that it is out of control on how much availability we have since it is based on the amount we receive. Thank you for your time!
