This survey has been developed by the United Service Group. We are a pro-choice group in Queensland supporting workers in front line industries, such as health, education, firefighters, ambulance, policing etc and other industries such as aviation, mining, veterans etc. We have been working together and have created an ad, which you can view by clicking here.

This survey has been designed for students, workers, parents, business owners, and everyone in between. There is a section in the survey for parents to ask their children 12 years and over a set of questions. Please support them through this question as this data is vital.

Once the survey has been collated, we will make the data available to the public.
