What the mode column means. Anonymous means you cannot link a response to a person at all. Confidential means you can, but you only ever report in groups. Identified means names are attached and everyone knows it. Anonymity is a promise about how you report, not a checkbox you tick, so pick the weakest mode that still gets honest answers and then keep your word.

Security Awareness and Phishing Reporting Questions

Developer Experience Questions

Do not paste all 43 of these into one survey. Six to eight questions is a technology survey. Thirty is a research project, and people stop answering long before they reach the end of it. Pick one block, add a single open text question, and send it.

IT Support and Digital Workplace Surveys

Start here when the thing you are measuring is a support channel rather than a product. The chatbot survey asks whether the self-service layer actually resolved the request, which is the number that decides how many tickets reach a human. If you want to score the human ticket instead, that is a post-resolution CSAT survey and it sits with the customer satisfaction surveys. If your question is broader than one tool, and you are really asking how staff feel about the whole digital workplace, use the employee feedback surveys instead.

User Acceptance Testing Survey Templates

This is the one survey on the page with a yes or no decision attached to it. A UAT survey collects structured tester sign-off before a release, so it runs identified rather than anonymous. You need to be able to chase a blocker back to the person who hit it. It does not replace your defect tracker. It answers the question a tracker cannot: whether the people who have to live with this build think it is ready.

Software Evaluation Survey Templates

Use this when the decision is buy, renew, replace or roll out further, and you need evidence from the people who actually use the tool. It differs from the UAT survey in timing and in stakes. UAT asks whether one build works. A software evaluation asks whether a product is worth keeping, so it includes the questions that expose workarounds and deal breakers rather than a simple pass or fail.

UX and Usability Survey Templates

Three templates that all measure an interface, split by what you are looking at. The UX Design Survey is the general purpose one and covers navigation, errors, readability and task success across a whole product. The Dashboard survey narrows to data products, where the real failure mode is not confusion but distrust of the numbers on screen. The Mobile Banking survey is the one to copy for any high stakes consumer app, because it also handles reliability, security perception and the questions you must not ask.

Security Awareness and Phishing Surveys

Both surveys measure what people believe and report, not what they would actually click. The Phishing survey is narrow and goes deep on one behavior: whether staff can find the report button, and whether anything stops them pressing it. The Security Survey is wider and covers passwords, screen locking, external sharing and whether your own controls are quietly getting in the way. Pair either one with a real simulation. If what you need to know is whether the training worked, the training evaluation templates handle the before and after.

Developer Experience Survey Templates

A DevEx survey is a technology survey pointed inward at your own engineers. It is modular on purpose, so you can run the full set once a quarter or fire a single module at a specific pain point such as CI reliability or onboarding time. Run it anonymously. The useful answers are the ones about process friction that nobody wants their name attached to.

What The Numbers Say

Most pages tell you which questions to ask and stop there. The harder problem is reading the answer. Here is what a normal score looks like for each of these instruments, with the source and the year attached, so you can tell whether your result is a problem or just a Tuesday.

How to score a usability block without pretending it is a percentage

The System Usability Scale is ten statements answered on a five point agree scale. For items 1, 3, 5, 7 and 9 the score contribution is the scale position minus 1. For items 2, 4, 6, 8 and 10 it is 5 minus the scale position. Add the ten contributions and multiply by 2.5, which gives a number from 0 to 100.[8]

That number is not a percentage and it is not a grade out of 100. A score of 68 is the average across roughly 500 studies, so 68 is a C, not a near miss.[1] Around 80 puts you in the top tenth, and 73 puts you in the top third.[2] Teams routinely report a 70 as a failure because it looks like a bad exam result. It is slightly above average.

Technology Survey FAQ

What questions should I ask in a technology survey?

Ask about one system, one audience and one decision. A workable set is a task success question, two or three rating statements about ease and speed, one question that surfaces workarounds, and one open text box. The question bank above has 43 items grouped by exactly that shape, copied from our own templates. Skip anything you have no intention of fixing, because asking about it sets an expectation you will not meet.

How many questions should a technology survey have?

Six to ten. Every template on this page sits between 7 and 10 questions and takes 5 to 7 minutes. Long technology surveys fail in a specific way: the people with the most to say are the busiest, so a 30 question form quietly filters your sample down to the least representative respondents. If you need more coverage, run a short survey more often rather than a long one once.

What is a user acceptance testing survey?

It is a structured way to collect tester sign-off before a release. It records which workflows each person tested, which environment they were on, whether they got through the workflow end to end, what blocked them if not, and whether they think you should ship. It is not a defect tracker and it does not replace one. It answers a question a tracker cannot answer, which is whether the people who have to live with the build believe it is ready. What should a UAT feedback form include?

Four things most teams leave out. A role question, so you can tell a finance tester from a warehouse tester. An environment question, because a surprising share of disputed results turn out to be two people on two different builds. A blocker question with defined severity levels, so the output is sortable. And an explicit go or no-go question, because a form that produces only ratings leaves someone else to make the call.

How do I write IT help desk survey questions?

Keep it to three to five questions sent right after the ticket closes, and anchor the first one on resolution rather than politeness. Someone can be charmed by an agent who did not fix anything. First contact resolution is the biggest single driver of service desk satisfaction, so ask whether the issue was solved and whether it took more than one contact.[4] For the self-service layer in front of the queue, the Chatbot Survey covers the same ground for a bot.

What is the System Usability Scale, and is it free to use?

It is a ten item questionnaire from 1986 that produces a single score between 0 and 100. It is free to use, but it is not public domain. The instrument carries a copyright line naming Digital Equipment Corporation and the year 1986, and Brooke states that it is freely available for use in usability assessment on one condition, which is that any published report acknowledges the source.[8] Reproduce it with that attribution and you are fine. Is 68 a bad SUS score?

No, it is exactly average. The number looks like a percentage and it is not one. Sixty-eight is the mean across roughly 500 studies, so scoring 68 means you are typical, around 73 puts you in the top third, and around 80 puts you in the top tenth.[1][2] Teams misread this constantly and go and rebuild something that was fine.

Should a technology survey be anonymous?

It depends on whether you need to act on individual responses. Security, DevEx and phishing surveys should be anonymous, because the honest answers are admissions of workarounds or mistakes. Software evaluations work best confidentially, so you can weight by role without exposing anyone. UAT should be identified, because a blocker you cannot trace to a tester is a blocker you cannot reproduce. The chooser table above marks the mode for every template.

How many responses do I need for a software pilot survey?

Treat a pilot as a census and invite everyone in it, then judge the result by response rate rather than raw count. As a rough floor, five people will surface most of the obvious usability problems in a qualitative round, but you want at least twenty before you report anything as a number.[6] Below that, one grumpy respondent moves your average enough to change a decision.

What is a DevEx survey?

A survey that measures how much friction your own engineers hit while shipping: environment setup, access and permissions, CI reliability, deploy confidence, documentation, on-call load and context switching. It is worth running because tooling sentiment is measurably volatile right now, with 84 percent of developers using or planning to use AI tools while only 3.1 percent highly trust what those tools produce.[7] Our DevEx Survey is modular, so you can fire one section at one problem.

Can a phishing survey tell me our click rate?

No, and any page that says otherwise is selling you something. A survey measures what people believe, remember and are willing to report. Click rate is behavior and only a simulation measures it. For scale, the 2025 baseline phish-prone rate across 62,400 organizations was 33.1 percent, dropping to 4.1 percent after a year of training.[5] Run both: the simulation tells you who clicked, the Phishing Awareness Survey tells you why nobody reported it.

What is the difference between a UX survey and a usability test?

A usability test watches a handful of people attempt real tasks, and five participants per round will expose most of the serious problems.[6] A UX survey asks a much larger group what they remember afterwards. The test tells you what broke, the survey tells you how widespread it is and whether it is getting better. They answer different questions, so if you only have budget for one, run the test first and survey once you know what to ask about.

How these templates are built

Every template on this page is a live survey in our own system, not a PDF or a screenshot of one. The question counts and completion times in the chooser table are read from each template page rather than estimated. The question bank is copied word for word from those same templates, so the wording here is the wording the survey actually sends. We write our own items in plain language, keep each template between 7 and 10 questions, and label the answer scale on every one.

Instrument licensing, and what the SUS licence actually says

Some well known instruments cannot be reprinted. The Gallup Q12 items are copyrighted and cannot be reproduced outside a participating company without permission, and the Utrecht Work Engagement Scale is free for academic use only. The System Usability Scale is a different case, and we checked it rather than assuming. The scale carries a copyright line naming Digital Equipment Corporation and the year 1986, and John Brooke states in his own chapter that it has been made freely available for use in usability assessment, with one stated prerequisite: any published report should acknowledge the source.[8] So it is free to use with attribution, not public domain, and the claim you will see repeated online that Brooke released it without copyright restrictions contradicts the line printed on the instrument itself. Our UX Design Survey carries the full ten item block with that attribution. We do not reprint it here as well, because you should read it inside the survey you are about to send.

Limits

The benchmarks above are borrowed reference points, not your numbers. The service desk satisfaction figures are 2012 data and are the most recent freely published help desk benchmark we could verify; the current ones are behind a paywall.[4] The phish-prone percentages come from simulated attacks rather than surveys, so a survey score cannot be compared against them directly. Usability averages are pooled across many products and industries, and yours can sit above or below for reasons that have nothing to do with quality. None of these figures can tell you whether a change between two of your own rounds is real, because that depends on your sample size and response rate, which only you can see.
